REGISTRATION IS LIMITED TO FIRST 500 APPLICANTS ONLY

• Registration is open to all from age 15 (born on or after 30/June/1971) to 40 years (as of 5/July/2011)

• Your registration fee includes (from June 30, 2011 - July 5, 2011) residence at UBC Gage Towers, breakfast, lunch & dinner, all seminars and all activities including outdoor excursions and social activities organised by the 5th World Zoroastrian Youth Congress Committee (5WZYCC) of British Columbia

• One application per attendee must be completed

• Legal guardian must complete this form for all minors (age 18 and under)

• Each application must be enclosed with 2 (two) recent colour passport size photos which must be endorsed with applicant’s signature and full name on the reverse

• Please check www.zsbc.org periodically for updates related to the 5th WZYCongress2011.
